We are committed to provide a effective and efficient service for all patients with back problems and spinal conditions.
Our triage process and referral criteria have been designed to reduce the waiting time for our patients and provide the most appropriate care pathway. Our bio-psychological assessment tools enable the team to quickly identify urgent and complex patients and provide bespoke care plans.
We are tracking the success of the service and individual outcomes at every interaction to expand our existing research database and ensure we adapt and improve to meet patients needs.

Our aim is to bring together all healthcare providers and stakeholders of spinal conditions working in North Hampshire. You are all a part of the Hampshire Backs network. We aim to support you as clinicians and give you the tools and instructions to provide effective and appropriate treatments for patients.
HAMPSHIRE BACKS QUESTIONNAIRE
This questionnaire is for GPs and Physiotherapy providers who are involved in our spinal pathway. This questionnaire must be sent along with a referral letter in order to be triaged in the new service.
The initial questionnaire is to be filled out by the patient before being sent to triage.
The post treatment questionnaire is to be filled out by the patient at the end of treatment.
